When Prison Break first aired, Dr. Sara Tancredi (played by Sarah Wayne Callies) became the emotional core of the series — the compassionate prison doctor who fell in love with Michael Scofield. So when news of her supposed death broke between seasons, fans were left devastated and furious. What Happened? At the end of Season 2, Sara and Michael are on the run from The Company. In the Season 3 premiere, Michael is incarcerated again — this time in the brutal Sona prison in Panama. He receives a box containing what appears to be Sara’s severed head. The visual is brief but shocking: a bloodied head with her signature auburn hair.

Michael breaks down. So did the audience. The “death” was a result of behind-the-scenes contract disputes. Sarah Wayne Callies was written off due to creative and scheduling conflicts (she was pregnant at the time). The producers chose a graphic death to raise the stakes for Michael and Lincoln.
